How do I apply to have my name and/or address changed on my current Provisional/Full Driving Licence ?

You need the following: (1) Current Provisional/Full Licence. (2) Provisional/Full Licence Application Form with your new name and/or address, completed and signed. (3) 2 identical passport sized photographs, signed on the reverse. (4) A Marriage Certificate, if the change of name is due to marriage; A Birth Certificate, if the change of name is reverting back to a maiden name; A Solicitor's Letter, confirming the change of name, if the change is by Deed Poll. Note there is no fee required for this application.
