What do I need to apply for a first Learner Permit?

You need the following: (1) Driver Theory Test Certificate. (2) Provisional Licence Application Form, completed and signed. (3) Eyesight Report Form for 1st ever licensing. (4) 2 identical passport sized photographs, signed on the reverse side. (5) Fee [ 1 or 2 year € 15 ]. (6) A Medical Report Form may be necessary in certain circumstances, namely : When applying for a provisional driving licence in categories C, CI, D, D1, EC, EC1, ED or ED1; When the applicant is over 70 years of age; If the applicant has a medical condition. (7) Birth certificate or passport. (8) Cert of Registration/Passport with valid visa for non EU members. (9) Passport for EU Nationals.
