If you lose your Vehicle Documents or your Driving Licence you can apply for a replacement for a small fee.
Vehicle Documents
To obtain replacement documents, download form RF134 (pdf), complete the form and it witnessed by a member of the Garda Siochana at your local Garda station. Forward your completed form with the appropriate fee:
- Vehicle Registration Book - €12
- Vehicle licensing certificate - €12
- Tax Disc - €6
- Trailer Licence Card - €6.00
- Certificate of Roadworthiness - Light €3.00 or Heavy €7.00 to this office at the address below:

Driving Licences
If you misplace, loose or destroy your driving licence while it is still valid you may replace it by completing an Appication for issue of Duplicate Driving Licence or Provisional Licence, form D.800 which is available from this office or any Garda station. This form must be completed by you, witnessed by a Garda and forwarded to us, at the adress below, with two passport sized photographs and the fee of €10.00.
If your misplaced, lost or destroyed licence has expired then you must apply for a Renewal of Driving Licence, form D.401, or Renewal of Provisional Licence, form D.201. This application must be accompanied by a 'Declaration of loss of existing licence wallet' which is available from this office or any garda station.
