Living Well with a Long Term Health Condition

The Living Well programme is a free self management programme designed to offer support to adults living with long term health conditions.  This programme is a Slaintecare funded initiative which provides a series of free workshops that run online for six weeks.  Two trained facilitators run the workshops each week and at least one of the facilitators lives with a long term health condition.

Self management is what a person does every day to manage their long term health condition.  This may include making choices to improve health such as becoming more active, eating more healthily and stopping smoking.  It may also include areas such as monitoring symptoms, coping with the emotional aspects of the condition and communicating with healthcare professionals.  The Living Well programme has proven to be effective in helping people to manage their health conditions better.

Three programmes are due to commence on the dates below. Feel free to share these details widely to service users, family members or friends that have a long term health condition and would benefit from this course.  

  1. Tuesday, 21st March 2023 from 10.30am - 1.00pm each Tuesday for six weeks
  2. Wednesday, 22nd March 2023 from 10.30am to 1pm each Wednesday for six weeks
  3. Thursday, 27th March 2023, from 10.30am - 1.00pm each Thursday for six weeks
