Parent/Guardian Consent Form Online Events and Activities

Offaly Library Service facilitates events and activities through online platforms. These platforms may include but are not limited to Zoom, Facebook and YouTube.

This form seeks consent for your child/children to participate in the event/activity and utilize these online applications for virtual event purposes.
